Program of All-Inclusive Care for the Elderly (PACE)

Description	PACE programs provide a full continuum of care to their beneficiaries, including adult day care, home health care, primary care, all medical care, social services and a wide range of services to keep them in their homes as long as is safe. There are currently 114 certified PACE programs operating nationwide. This number is limited because there are many stringent requirement to become a PACE Program, including a significant investment of capital construction
Patient Population	Seniors age 55+ who need nursing home level of cares
Typical Lead	PACE programs are administered by PACE certified agencies
Role of Home Health	Home health agencies may apply to become certified PACE programs if they otherwise fit the criteria. They may contract with a PACE program to provide in-home skilled nursing and other medical services to PACE participants.
Reimbursement Model	Fully capitated PMPM. The program consolidates funding from Medicare and, when beneficiaries qualify, from Medicaid as part of a bundle of services in exchange for the full risk of the patient.
